How to Master Documentation

Documentation in nursing is the most critical aspect of nursing and can also be the most feared. I started teaching documentation because no one really teaches nurses how to chart. Truthfully nurses can be caught reading a history and physical for documentation or charting vague notes. Where do nurses learn how to chart what they have assessed? Every scenario can be documented. Remember utilizing the five senses will greatly improve documentation and confidence. What do you see? What do you hear? Is there a smell? Issues with taste? What do you feel? So let’s look at common diagnosis: Hyperglycemia. Remember record according to the actual date and time. What do you see? Dry skin, sticky mucus membranes, urinary frequency What do you hear? How is the patients speech, are they alert and coherent, tachypnea Is there a smell? Acetone, Taste? Has the patient been eating, is there a lost of appetite Touch? How does the skin feel warm or cool to touch is it intact Once those are established we are ready to document a quick note: 01/01/1999 0525am—————————- Patient O states “I have been vomiting and I feel fatigued and lightheaded”. Patient denies any pre existing diagnosis states he was diagnosed with “borderline diabetes” but has not been prescribed medication.Face is assessed flushed, oral mucosa is dry and sticky, skin is dry, ashen and tents. Tachypnea assessed and breath has an acetone odor. Blood pressure is 112/54, Pulse 84, respirations 33 and deep. Oral temperature is 99.3F. Blood glucose is 583mg/dl by fingerstick. Dr. V notified at 0538am. New orders received including 1000ml sodium chloride bolus. Regular insulin order for 10 units obtained given in right lower quadrant area subcutaneously as a stat dose.Second peripheral access started in left forearm 18gauge angiocath. Serum collected at this time. Explanation of interventions and rationales given to patient and family. See quick flow sheet for frequent vital signs and intake and output. Nursing assessment is ongoing. Remember the nursing practice act Once nurses have an abnormal finding go back and reassess. 01/01/99 0555——————————————- Patient states “I feel better…I do not feel lightheaded anymore. No emesis assessed. Patient denies pain, fall assessment completed, vitals and blood glucose reassessed, nursing assessment is ongoing. The notes do not have be perfect merely state the facts and what was done for the patient. The second assessment will also include urine output, educating and monitoring oral intake remember sometimes these patients have polyphagia. Nurses will document “will continue to monitor” Monitor what exactly? The cardiac monitor? Pulse oximetry monitor? These copycat statements can be detrimental later if the patient falls, declines in mentation or stability. The question then becomes I thought the nurse was “monitoring” what happened? Take your time. Use your five senses and write your own assessment findings.

Your first clinical rotation can be both exciting and nerve-wracking. Discover essential preparation tips, what to expect on your first day, and how to make the most of your clinical experience. 1. Research the hospital, where is it located read the reviews, parking, the population how many beds. 2. Be on time. Clinical rotation is as up close and personal as it gets. It is like going to work, nurses are ready to end their shift and give report. 3. Do not judge. You’re not always there do not assume a nurse is not approachable or threatening. They do not know you personally and they are working. 4. Respectfully ask questions. Theory and clinicals do not always align perfectly. Patients and patient situations vary. 5. Have your equipment. For example your stethoscope this is the time to really listen to lung sounds and assess. Be prepared and take notes. 6. Clinical's is the practical rotation not theory. Nurses can become disengaged when students are “studying” on their phones or in the break rooms instead of learning task and skills 7. Incorporate what you are currently studying and ask questions related to the subject. This is the time to get knowledge from nurses who are actively working some may have recently taken the NCLEX/NGN 8. Practice and engage like you are taking care of the patient as you soon will be 9. Watch the technicians many of the technicians are paramedics and medical students and they share a wealth of knowledge. 10. Smile. A warming smile will receive more from the clinical experience guaranteed.
